Service Fees & Enrolment
We welcome new patients to Devonport Family Medicine.
To enrol at our practice to receive subsidised consultations, you simply need to complete the enrolment form below and provide a valid form of ID. Alternatively, you can come into the practice to complete an enrolment form.
We are also happy to see new patients who may not be eligible for enrolment and patients who do not wish to make us their regular doctor at this time.

Service Fees
The below fees are to provide medical services to you. Please note a standard GP appointment is 15 minutes. Please be aware that if you cancel your Doctors appointment with less than 24 hours notice a cancellation fee of $30 will apply. The same fee applies for non-attendance of a booked appointment
Patients with a current Community Services Card (CSC) are entitled to reduced fees. Your CSC must be sighted at the time of your appointment to be eligible for reduced consultation fees.

How do I enrol?

Parents can enrol and sign for children under 16 years of age. Children 16 years or older must sign their own form.
It is a Ministry of Health requirement that all patients provide an original signed enrolment form and proof of identification, for example, passport and/or birth certificate and if necessary proof of residency status.
Casual Patients
If you are a visitor or wish to be a casual patient and attend this practice, simply click here to enrol and select the Casual or Visitor option.
Visitors and casual patients consultation fees are not subsidised by the government and so therefore patients are required to pay the full consult fee.
