We welcome new patients to Devonport Family Medicine
To enrol at our practice to receive subsidised consultations, you simply need to complete the enrolment form below and provide a valid form of ID. Alternatively, you can come into the practice to complete an enrolment form. We are also happy to see new patients who may not be eligible for enrolment and patients who do not wish to make us their regular doctor at this time.

How do I Enrol?
To enrol simply click here Enrolment Form (ScreEnrol requires a mobile or touch screen device to capture signature). If you would prefer a paper form please either pop in to reception or email [email protected]
Parents can enrol and sign for children under 16 years of age. Children 16 years or older must sign their own form. It is a Ministry of Health requirement that all patients provide an original signed enrolment form and proof of identification, for example, passport and/or birth certificate and if necessary proof of residency status.
If you are a visitor or wish to be a causal patient and attend this practice, please advise our receptionist. Visitors and casual patients consultation fees are not subsidised by the government and so therefore patients are required to pay the full consult fee.

Welcome to alert level 1
In alert level 1 we will maintain some important protocols to help prevent the spread COVID-19. If you have respiratory, cold or flu-like symptoms please let us know when you book your appointment so we can prepare for you coming into the surgery by ensuring there is a room available away from other patients. A two metre distance between patients in the waiting room will be maintained as will the hand sanitising and mask station as you walk in to reception area.
COVID SWABBING – if you think you need a COVID-19 swab, please phone the practice. If you are symptomatic, you will be swabbed in your car. If you have any questions, please don’t hesitate to call the practice on 09 445 0528.
