Fees & Enrolment

We welcome new patients to Devonport Family Medicine
To enrol at our practice to receive subsidised consultations, you simply need to complete the enrolment form below and provide a valid form of ID. Alternatively, you can come into the practice to complete an enrolment form. We are also happy to see new patients who may not be eligible for enrolment and patients who do not wish to make us their regular doctor at this time.

Fees

Registered Patients

  • Adults 65+ years – $48
  • Adults 25 – 65 years – $50
  • Youth 18 – 24 years – $50
  • Junior 14 – 17 years – $43
  • Child under 14 years – n/c
  • Nurses Consultation – $25

Non-capitated Fees

  • Adults – $80
  • Youth 18 – 24 years – $80
  • 14-17 years – $60

Home Visits

  • Local Area – $85
  • Outside Local Area – $105

ACC Patients Fees

  • Adults – $30
  • Adults 65+ years – $30
  • Junior 14+ years – $30
  • Under 14 years – n/c
  • Community Services Card – $18.50

Prescriptions

  • Regular – $15
  • Fax – $20
  • Urgent/ Same Day – $25
  • Community Services Card Holders – $10

How do I Enrol?

Complete the Enrolment Form and return the original signed form to us. Parents can enrol and sign for children under 16 years of age. Children 16 years or older must sign their own form. It is a ministry of Health requirement that all patients provide an original signed enrolment form and proof of identification, for example, passport and/ or birth certificate and if necessary proof of residency status.

Casual Patients

If you are a visitor or wish to be a causal patient and attend this practice, please advise our receptionist.  Visitors and casual patients consultation fees are not subsidised by the government and so therefore patients are required to pay the full consult fee.
